How to fill-up the DS-160 form
Here are the answers to some common issues we face during filling up the DS160 visa application form. Thanks to Mahmudul Hasan Razib for providing this information. This helped me to fill up my DS160 so I thought I create a doc and save the comment post for others here :
i. Full Name in Native Alphabet .

does not apply
ii. National Identification Number
--- national id number
iii. Home Address
--- passport permanent address
While filling up the address, / is not allowed. use hyphen in that case. for example, if your address is 2/A, use 2-A
iv. Passport Book Number
--- does not apply
v. Have you made specific travel plans?
--- NO
intended arrival date before at least 2 week before the recommended date at your I20. length of stay 5 years (as per your I20)
vi. Address Where You Will Stay in the U.S.
--- School Adreess from your I20.
vii. Contact Person or Organization in the United States
--- School Official information from your I20
viii. Do you have any other relatives in the United States?
--- better to say no if there is no immediate relatives
ix. Monthly Salary in Local Currency (if employed)
--- Gross Slalry in Taka
x. briefly describe your duties
--- Describe your position and duty in few lines.
xi. Have you attended any educational institutions other than elementary schools?
--- Enter university information with street address and then give College address (e.g. NDC)
xii. Date of attendance from: to
--- Start of your class in university to result publish day as mentioned in your certificate
xiv. Do you have a communicable disease of public health significance such as tuberculosis (TB)?
--- No
xv. You have indicated that you will be studying in some capacity while in the United States. List at least two contacts in your country of residence who can verify the information that you have provided on this application. Do not list immediate family members or other relatives. Postal office box numbers are unacceptable.
--- Colleagues or Friends or Teacher who can verify you if DHS communicate with them.
